STAT RN Patient Advocacy & Healthcare Navigation

 

Healthcare can become overwhelming quickly. A hospitalization, new diagnosis, multiple specialists, medication changes, or a complicated appointment can leave patients and families wondering what happened, what questions to ask, and what comes next. STAT Health & Wellness provides private-pay RN patient advocacy and healthcare navigation to help individuals and families become more organized, informed, and prepared as they move through the healthcare system.

 

How an RN advocate can help

Prepare for medical appointments and organize the questions you want answered.

Create and maintain an organized medication, provider, and health-information summary.

Take notes and help you keep track of provider instructions, tests, referrals, and follow-up plans.

Help you organize hospital discharge information and prepare questions for your healthcare team.

Assist families navigating care involving multiple physicians, specialists, appointments, and records. Help adult children or caregivers stay organized when supporting an aging family member. Communicate with healthcare providers when you have given appropriate authorization.

 

Your advocate works for you

STAT's role is to support your voice, your questions, and your ability to navigate your healthcare. The goal is not to make medical decisions for you, but to help you understand the information you receive, identify unanswered questions, and communicate effectively with your healthcare team.

 

Important scope statement

STAT RN Patient Advocacy & Healthcare Navigation provides nursing education, advocacy, organization, communication support, and healthcare navigation. Services do not include medical diagnosis, prescribing, or independently changing a treatment plan. STAT does not replace your physician, nurse practitioner, pharmacist, therapist, home health agency, emergency department, or other treating provider. STAT is not an emergency service. Call 911 or seek appropriate emergency care for a medical emergency.

RN Advocacy Consultation

Review current healthcare situation, goals, concerns, provider list, and next-step organization.

$100 | 60 minutes

Appointment Prep & Review

Prepare questions, medication/provider list, priorities, and review provider instructions after the visit.

$125 | 75 minutes

Hospital-to-Home Navigation

Organize discharge instructions, medication changes as ordered, follow-ups, referrals, equipment needs, and questions for providers.

$200 | 120 hours

Additional Advocacy Time

Additional navigation, record organization, calls/coordination, authorized by the client, or follow-up.

$100 | 60 minutes
